Coaches Application

WE NEED HEAD COACHES
Please fill out an application to be a Head Coach.  Our programs must have Head Coaches to get started.  All Head Coaches qualify for a $75 Head Coach Voucher that can be used toward your next youth sports registration fee.

Registration Information and Forms will be available January 11th.

PROOF OF RESIDENCY REQUIRED (Must bring at least one of the items below)
Town Water Bill, Town Property Tax Bill/Receipt or MLGW Bill 

PROOF OF AGE REQUIRED (Must bring at least one of the items below)
Birth Certificate or Medical Shot Record with Participant’s Birthdate

 Register at the Collierville Community Center located at 440 West Powell Road
Monday - Friday * 8:00 am to 5:00 pm
